Fix up doc linkages

Summary

Minor doc site tweaks for the migration guide.

Details

After looking at the nightly publication at https://vllm-project.github.io/guidellm/main I've made a few minor tweaks:

• Added the migration document to the Guides index page
• Changed the title of the document to clarify the context

Neither of these are probably worth delaying release on Monday morning -- but if anyone happens to review over the weekend and we can get it in, that'd be good.

Test Plan

I built/deployed a local mkdocs server to check the appearance.
