refactor(eve): construct instrumentation at channel delivery

Summary

• Resolve channel audience at delivery into one serializable drop/record decision.
• Construct the harness hooks, telemetry settings, integrations, and trace callbacks from that decision.
• Remove channelAudience from harness lifecycle events, attempt scopes, trace state, and span export context.
• Persist only the decision for turn continuations; the harness receives neither audience nor the decision.

This intentionally limits the change to the audience-to-harness boundary. It does not add a general execution-scope framework, wrap unrelated durable operations, change dynamic resolver channel metadata, or bump the channel extension epoch.

tracePolicy now returns { action: "drop" } or { action: "record", recordInputs, recordOutputs }. Production defaults still record public conversations and drop private/unknown conversations; local tracing continues to admit public/unknown conversations.

Follow-up to #2335. Replaces #2438.

Additional Suggestion:

On a cancelled first-delivery turn, InstrumentationDecisionKey is dropped from the preserved serialized context, so the cancellation epilogue reconstructs UNINSTRUMENTED and never closes provider operations opened by the discarded step nor emits turn.cancelled instrumentation.
