A clear evaluation plan/results

• We need a clear evaluation plan, including what the baseline, dataset, metrics, etc. are.
• We want to see the baseline approach's numbers and our numbers.

Added a clear evaluation plan + live results: EVALUATION.md — dataset, control/treatment methodology, metrics (fix rate, oracle-confirmed rate, attempts, token counts), verification methodology, and current numbers (14/30 bugs on each arm as of now, updated as the run progresses).

Leaving this open until the full 30/30 pass completes on both arms.
