Any fragment in `href` is extracted and injected into `locations.fragments` on construction, to protect against incorrect Locators. Explicit `locations.fragments` take precedence when already set. Simplifies `Link.locator` and `Manifest.locatorFromLink()` which were pre-splitting to work around the gap.

This updates the timeline so that it can handle HTML (id, scroll, position).
It adds an
augment()method toTimelineso that Navigators can augment the timeline e.g. update items from the positions list provided onEPUBNavigator.load().
