The foundational CI/CD repair, built on top of #1338 (fast-forwarded verbatim — all 31 commits preserved), so this PR supersedes it and #1338 will close automatically when this lands.
State of play (measured, live, 2026-10-25)
Required-context gate: inspected=true, 23 producible + 1 app-owned, code_coverageVERIFIED by a successful upload run. One finding remains: rule:code_quality in ruleset 23787415 — Code Quality is required by the main gate but not enabled in repository settings, so no result can exist. No file in this tree can fix that; it is declared in the shrink-only allow-list with the owner decision named (enable Code Quality orscripts/remove-unsatisfiable-data-rules.sh --apply --rule code_quality). The allow-list's context list is still empty, as Retire the last exemption: the required-context allow-list is now empty #1338 established.
Two gate defects fixed with fixtures: a 404 producer-runs lookup (the normal state of the PR introducing a workflow) was an inspection failure; gh < 2.32 (no --slurp) turned a readable repo into inspected:false.
…empty
The ruleset was repaired at the decision layer, so the context this file
declared unsatisfiable is no longer required at all. Keeping the entry would
have been a fabricated debt: the gate classifies it as stale and warns, so it
could not break anything, but the file's own preamble says removing the last
entry is the goal rather than an edge case.
Header-only now, with the retirement recorded rather than the line deleted:
which context, why it could never be produced, what it cost (#1336: 65 green
checks, cleared review, MERGEABLE, BLOCKED), and the four ruleset changes that
retired it. A declaration that vanishes without a trace is indistinguishable
from one that was never true.
Verified against the live ruleset: inspected=true, producible=23, app-owned=1,
zero findings, so reconcile reports 0 findings / 0 undeclared / 0 stale.
Audit addendum records the verified after-state, including the timestamp that
settles the argument: #1337 carried this repair and merged at 21:38:20Z, the
ruleset was corrected at 22:57 — the fix provably did not enable its own
merge.

…d look
The ruleset repair changed what is true about this repository, and the
documentation still described a world where a required check could not be
produced and a bypass was the only way through.
README — new 'What actually blocks a merge' section. States the 21 required
checks, strict up-to-date and no bypass actors, then explains what a gate is
NOT: a required check no PR can publish makes every PR undecidable rather than
stricter, with #1336's 65-green-but-BLOCKED as the worked example. Names the
three defences (producibility gate, declared shrink-only exemptions, scripted
repair).
EXPLAINME — two durable decisions added alongside the existing ones: a required
check must be producible on a pull request, and no standing bypass on the
default branch.
wikis/maintainers — a new guardrail: the merge decision layer is part of the
standard, not infrastructure someone remembers to configure. Includes the
all-or-nothing nature of bypass and the narrow-instrument rule for editing
protection.
wikis/developers — replaces the 'decode a red gate' TODO with the actual
diagnosis. Green everywhere plus BLOCKED is a missing required check, not a slow
runner, and re-running cannot create it. Copy-pasteable commands.
coordination.k9 — two critical invariants (required-checks-are-producible,
no-standing-bypass) and two reflex guards (pull_request, ruleset-edit). Passes
.validate-k9.sh: 12 conforming, 0 grandfathered.
0-AI-MANIFEST.deed — invariant 50: never merge through a bypass to get past a
required check, and downgrading the gate (continue-on-error, || true, a new
allow-list line) is not a repair. Parentheses and quotes verified balanced; the
Python deed_lint port cannot parse this file before or after the change (#u8 is
unsupported by that port, pre-existing).
CICD-WORKFLOW-CATALOG — fork-governance.yml and required-contexts.yml were both
missing entirely. fork-governance is what publishes the context the ruleset now
requires, so its absence from the catalog was a real gap. Also flags
runtime-policy.yml as retired rather than silently listing a workflow that no
longer exists.

…m real
CodeRabbit reviewed #1338 and caught something that was not a documentation nit:
the audit named four rulesets carrying `bypass_mode: always`, and the repair
removed it from one. Measured after the repair, three still had it.
Sorting them by whether it matters:
24255701 Require-Linear-History required_linear_history REAL GAP
24587723 Lock-Floor 2 status checks duplicated
24309995 Secret-Scan-Floor 1 status check duplicated
Lock-Floor and Secret-Scan-Floor require contexts that 23787415 also requires,
and 23787415 now has no bypass, so their bypasses discard nothing that is not
still required next door. required_linear_history is the only rule in the
repository that lives in exactly one ruleset, so its bypass is the entire
difference between append-only merging being enforced and being advice. That is
the correction that mattered: the earlier write-up implied the bypass was gone.
scripts/remove-ruleset-bypass.sh removes it, following the narrow-instrument
pattern: report-only by default, skips non-active rulesets rather than writing
to them, asserts the payload changes nothing but bypass_actors, and verifies the
read-back. 19 offline fixtures with an injected gh cover every branch.
Also fixes the audit's context-count scoping: the applied table is one ruleset
(21 contexts), the gate's producible=23/app-owned=1 is the whole inspected set,
and 24 rule-entries resolve to 21 distinct context names because the duplicate
rulesets restate three of them.
Corrects '80 minutes' to 78, and narrows the allow-list's empty-file claim: it
asserts no repository-owned required context is a finding, not that every
required context is satisfiable. app-owned contexts are excluded by design.

Codex found this on #1338 and it is a real bug, in the same family as everything
this session has been about: the verification step could report verification it
never performed.
"$GH" api ".../rulesets/$id" >"$tmp/verify.json"
v_bypass="$(jq -r '.bypass_actors|length' ...)"
if [ "$v_bypass" -ne 0 ]; ...
If that GET fails, verify.json is empty, jq yields an empty string, and
`[ "" -ne 0 ]` is a *syntax* error that evaluates false. The mismatch branch is
skipped, and the script prints 'verified: 0 bypass actors' and exits 0 — after
writing to live branch protection and confirming nothing. A gate that reports a
check it never ran, in the one script whose entire job is to not do that.
Both scripts had it, including fix-required-context-name.sh, which shipped in
#1337. Both now require the GET to succeed, require the response to be a ruleset
payload, and require numeric counts before comparing.
Fixtures added that fail against the old code and pass against the new: a GET
that returns 521 for the read-back, and a GET that returns valid JSON of the
wrong shape. Verified non-vacuous by reverting the script: 4 failures, restored:
24/24.

Third instance of the same defect in one day, and the gate could not see this
one at all.
GitHub evaluates code_coverage and code_quality itself. They never appear as a
status context, so check-required-contexts.sh — which traverses
required_status_checks — reported a clean repository while the main gate held
two rules nothing here can satisfy: code_coverage wants minimum_coverage 80 and
the only coverage job in this tree reports SKIPPED; code_quality wants severity
'all' and no code-quality results are uploaded.
Neither rule is in this repository's own config/rulesets/*.json. They arrived
through a UI edit, which is why nothing reviewed them: a ruleset edited by hand
has no diff.
The gate now reports rule types that depend on uploaded data as
UNSATISFIED-RULE findings, named 'rule:<type> in ruleset <id>' so they flow
through the same report, JSON and allow-list reconciliation as contexts. The
workflow counts the new verdict as a finding.
Verified against the live ruleset: UNSATISFIED-RULE=2, both named, 0 undeclared
false negatives. Context checks are unchanged (producible=23, app-owned=1).
Co-authored-by: arena-agent <297053741+arena-agent@users.noreply.github.com>
A bare name is a status context nothing publishes; a 'rule:' entry is a rule
whose data nothing uploads. They fail identically and are repaired differently,
so the message now says which is which — and says plainly that declaring one in
the allow-list is the last resort rather than the fix.
Co-authored-by: arena-agent <297053741+arena-agent@users.noreply.github.com>
Recorded before building anything, because the alternative was plumbing that
uploads 0%.
code_coverage (minimum 80) and code_quality are the fourth and fifth instances
of the same defect, and unlike the first three no file in this repository can
satisfy them: both need GitHub Code Quality enabled (a repository setting) and
either a Cobertura upload with code-quality: write or the CodeQL - Code Quality
check on pull requests.
Measured against what the rules judge: 756 .adoc, 245 shell scripts, 90 shell
tests, 8 Rust files across five crates with no test files, and no coverage
instrumentation in CI. An 80% line-coverage floor there is not reachable by a
code change, and the number would describe scripts/ rather than the artifact
this repo produces.
Two honest options, both settings: remove the rules, or enable Code Quality,
add the upload, and set a floor the tree can meet — noting the coverage rule
does not wait for uploads, so the uploading job must itself be required.
Leaving them as they are is the one option that is not: it fails every otherwise
perfect pull request, which is what this audit exists to document.
Co-authored-by: arena-agent <297053741+arena-agent@users.noreply.github.com>
Codex found this on #1338 and was right. data_rules_json appends findings
straight to $ROWS, so they never pass through the per-context case that
increments $unsatisfiable — and the arm I had added to that case was dead code,
sitting inside a loop those rows never enter.
Consequence: with REQUIRED_CONTEXTS_STRICT=1 and a data-dependent rule as the
ONLY finding, the audit exited 0. The gate would print the reason a pull request
cannot merge and then declare success — the exact defect it exists to catch,
committed by the gate itself, for the third time in this session.
Rows are now counted after the loop, with an explicit numeric guard, and each
one is reported as an error in strict mode.
Verified against the live ruleset, where these are the only findings:
advisory rc=0, strict rc=1.
Co-authored-by: arena-agent <297053741+arena-agent@users.noreply.github.com>
The main-gate ruleset carries `code_coverage` (minimum 80%, maximum drop 5%)
and `code_quality`. Neither is declared in config/rulesets/*.json, which is how
they got here: added through the UI, never wired to anything that produces data.
A coverage rule evaluates only reports that have already been uploaded, and it
does not wait for one. So both rules were unsatisfiable, and the audit recorded
that as "unreachable by writing code".
That conclusion was too broad, and this commit is the correction. It is true for
the 245 shell scripts and 756 AsciiDoc files, which have no measured surface
here. It is not true for the Rust code: this tree holds six independent crates,
each with tests, and cargo llvm-cov can measure them today.
So rather than delete the rules, make them mean something: run the measurement,
upload Cobertura, and let the ruleset see real data.
Details that are load-bearing and look optional:
- No `paths:` filter. This job becomes a required status context, and a filtered
workflow never reports on a pull request that misses the filter — the required
check then waits forever. Same trap as lock-sync-gate.yml.
- Both `push` and `pull_request`. Code Quality compares the pull request against
the default-branch baseline; with no push trigger on main there is no baseline.
- No `workflow_dispatch`. The upload action needs a pull request number on a
non-default branch and skips what it cannot attribute, so a manual run would
look like it worked and publish nothing.
- Upload labels come from a stable matrix `name`, not `strategy.job-index`.
Inserting a matrix entry would renumber every report and silently reset the
history comparisons the rule depends on.
- `--locked` on `cargo install` (version-pinned tool: a coverage tool that
changes under you changes the number the rule is measuring) but deliberately
not on `cargo llvm-cov` — four of the six crates commit no Cargo.lock.
- `fail-on-error` left at its default. A coverage job that goes green without
uploading is the exact defect this file exists to prevent.
Two things this does not do, stated here because a coverage number that quietly
excludes most of the tree reads as a guarantee it cannot keep: shell and AsciiDoc
are unmeasured, and the 80% floor is still a number nobody measured — it must be
replaced with a reachable one taken from the first real run.
Co-authored-by: arena-agent <297053741+arena-agent@users.noreply.github.com>
Two things the first run taught, both recorded in the file.
The job name was `Rust coverage` with a matrix underneath, so GitHub generated
the context as `Rust coverage (<crate path>, <slug>)`. A required status context
is matched by string equality and has to stay stable for as long as the ruleset
requires it; baking `.verisimdb/ecosystem-ingest` into that string would make the
crate's directory layout part of the branch protection. The name now uses the
slug alone: `Rust coverage (ecosystem-ingest)`.
The measured percentages went only to `$GITHUB_STEP_SUMMARY`, which is reachable
from the run page and nowhere else. A floor chosen from a number nobody can
re-read later is a floor chosen from memory, so each crate now also emits a
notice annotation, which appears on the pull request and is readable through the
API. A crate that instruments no lines warns instead of contributing a
misleading 0.00%.
Also records the first run as it happened: six legs measured, six uploads failed
404 because Code Quality is not enabled for the repository. The job went red
rather than green-without-uploading, which is the point.
Co-authored-by: arena-agent <297053741+arena-agent@users.noreply.github.com>
A percentage cannot set a floor. GitHub compares the rule against a weighted
aggregate across every uploaded report for the branch, so the floor has to be
chosen knowing how much of that aggregate each crate carries — 46.92% on a
crate holding two instrumented lines is not the same finding as 46.92% on a
crate holding two thousand.
The notice and the step summary now carry `covered/valid` alongside the ratio,
which makes the aggregate computable from the pull request without re-running
anything, and makes a crate that instruments nothing visible as `0/0` rather
than as a suspiciously round zero.

…e measurement
The audit said `code_coverage` "cannot be repaired in this repository by any
change to this repository's files", and printed a table saying the Rust surface
was 8 files across five crates, none with a test file. Two of those three facts
were wrong, and the conclusion rested on them.
Measured: 45 `.rs` files, 14 `Cargo.toml`. Outside `9-archive/` that is 8 files
across six crates, and every one of the six has tests. The count of 8 was right
for the non-archive tree and then stated as the whole tree.
The correction records the real figures from run 40146453852: 1336 of 2111
instrumented lines covered, an aggregate of 63.29%. The 80% floor is 16.71
points above what the tree meets, so it is a permanent block rather than a bar,
and the reachable figure is at or below the aggregate — after which `max_drop: 5`
is what actually enforces something.
Three limits are written down with the number, because a coverage figure that
hides its scope reads as a guarantee it cannot keep: it covers six crates and
not the 245 shell scripts or 756 AsciiDoc files; it excludes eight archived
manifests on purpose; and it is undefined until `main` uploads a baseline.
What survives from the original subsection is `code_quality`, which genuinely
needs a repository setting and no file can supply it.
Co-authored-by: arena-agent <297053741+arena-agent@users.noreply.github.com>
Both data-rule messages asserted that the repository "never uploads". As of this
branch that is false for coverage: `.github/workflows/code-coverage.yml` uploads,
and its six legs measured successfully. An error message that is confidently
wrong is worse than a vague one here, because it sends the reader looking for an
uploader that already exists instead of at the setting that is actually missing.
The verdict does not change. A rule with an uploader in the tree is still
unsatisfiable until Code Quality is enabled for the repository — GitHub's
ingestion endpoint returns 404 until then, which is what the first run hit — so
it stays a finding and strict mode still exits 1. Only the sentence changes, and
it now names which of the three repairs applies:
- `code_quality`: enable the feature; no file can supply it.
- `code_coverage` with a producer: the upload is happening and being rejected.
- `code_coverage` with no producer: nothing uploads it; write the uploader or
remove the rule.
Two tests, one of which is a negative control. The positive case alone would pass
for a script that never read the tree at all, which is the failure mode this
session has already hit twice.
Co-authored-by: arena-agent <297053741+arena-agent@users.noreply.github.com>
Codex, on this file, and correct. `actions/checkout` on a `pull_request` event
defaults to GitHub's synthetic merge commit, but `actions/upload-code-coverage`
attributes the report to `github.event.pull_request.head.sha`. When a head is
behind its base those are different trees, so base-branch changes would be
counted in a coverage figure recorded against a commit that does not contain
them — quietly, and in the direction that matters least: it would make a stale
branch look better than it is.
The checkout now pins the head sha, falling back to `github.sha` on `push` where
there is no pull request object. Measurement and upload finally describe one tree.
Co-authored-by: arena-agent <297053741+arena-agent@users.noreply.github.com>
Codex flagged this on #1338 and was right about the shape of the problem: the
audit emitted UNSATISFIED-RULE for a data-dependent rule the moment one was
present, so the gate that is supposed to prove the repository is mergeable would
have gone on failing after the repository became mergeable. A permanent alarm is
how people learn to ignore the alarm.
The distinction now rests on evidence, and evidence is an OUTCOME:
- `code_coverage` is satisfied when the uploading workflow has a COMPLETED RUN
THAT SUCCEEDED. A workflow merely existing proves nothing — every run on this
branch so far failed with HTTP 404 while Code Quality was off, and a check that
accepted file presence would have called that satisfied and waved the block
through. `an uploader with no successful run is not evidence` pins that case.
- `code_quality` is satisfied when GitHub has registered the analysis workflow
for the repository. There is no uploader in the tree to inspect; the feature
is the only source.
A satisfied rule keeps its row as VERIFIED-RULE and is reported with its
evidence, so the audit still answers "was this rule considered?". Only the
verdict changes.
Three ways this could lie, and what happens instead: a malformed or failed
response is an inspection failure (`inspected: false`), never "no data" and never
"data present"; the resting fixture state is empty evidence, so nothing passes by
accident; and each fixture holds one variable, so the coverage test cannot pass
because quality was satisfied.
18 fixture tests, including the four states and both failure modes.

Register the squabble chains job as a required check
When squabble chains exits 4 for a dependency cycle or dead upstream, this step makes the workflow red, but that red does not block merging: a repo-wide search finds no registration of cicd-squabbler.yml or its squabble / Squabble chains (cycles and dead upstreams block) context in config/rulesets/gates.json or another checked-in ruleset source, while the committed audit still describes the existing 21 required contexts. Add the hard chains context to the required gate (while leaving the fight-report job advisory), otherwise the workflow's stated hard-gate invariant is unenforced.
The reason will be displayed to describe this comment to others. Learn more.
Paginate the data-rule ruleset inventory
On repositories with more than 30 rulesets, this newly added data-rule audit examines only the first page, so a later applicable code_quality or code_coverage rule is omitted while the report still records inspected: true; the required-context gate can consequently pass without declaring that unsatisfied rule. GitHub documents a default page size of 30 for this endpoint, and gh api fetches additional pages only with --paginate, so combine all paginated arrays before iterating the IDs. 73-76: 🎯 Functional Correctness
No caller change is required.
.github/workflows/cicd-squabbler.yml grants contents: read, pull-requests: read, and attestations: read at the reusable-job level. It also forwards ADMINISTRATION_READ_TOKEN from secrets.HYPATIA_SCAN_PAT. The reported startup failure and missing-secret condition do not apply.

Select the renderer from the same checkout as the post job.
When the repository is metadatastician/standards, this step checks out the caller and skips the canon checkout. The render step at Lines 157–160 still selects .fork-governance-canon/scripts/fork-governance-render.sh for that slug. The file is absent, so rendering fails and the job does not publish the governance comment. Update the renderer selection to use the same repository-identity condition as the checkout steps.

Review comment at @.github/workflows/fork-governance-reusable.yml at line 108:
Update the renderer selection in the post job to use the same
repository-identity condition as the checkout steps, so
`metadatastician/standards` uses the renderer from the caller checkout rather
than the absent canon checkout.

Compare creation times as instants, not strings.
GitHub documents workflow created_at values with timezone offsets. For example, a workflow created at 09:00-08:00 is newer than a run created at 12:00Z on the same date. This string comparison accepts that older run as upload evidence and can mark code_coverage verified incorrectly. Parse and normalise both timestamps before comparison. (docs.github.com)

Review comment at @scripts/check-required-contexts.sh around lines 552 - 553:
Update the workflow-run filter comparing `.created_at` with `$since` to parse
and normalise both timezone-aware timestamps as instants before comparison;
preserve the existing repository filter and behaviour when `$since` is empty.

"comment": "The pons job is the one job in this reusable that uploads SARIF, using github/codeql-action/upload-sarif, and security-events: write is the only permission spelling that can perform that upload. It is job-scoped: the reusable workflow level and the panic-attack job remain contents: read, and the scan itself still fails closed on error-severity findings or an incomplete scan.",

Point the dismissal at the write permission
For this reusable, security-events: write is on line 71, while line 70 is contents: read. Because the dismissal matcher requires an exact path and line, the actual TokenPermissionsID alert will not match this entry; the newly added grounding check in scripts/tests/dismiss-code-scanning-alerts-test.sh also fails for this reason. Update this entry and its fixture to line 71.
echo "::warning title=No instrumented lines: ${{ matrix.name }}::cargo llvm-cov reported no instrumented lines; this crate contributes nothing to the aggregate."
The reason will be displayed to describe this comment to others. Learn more.
Fail coverage legs that instrument no lines
When a matrix crate produces zero instrumented lines—for example after a configuration change excludes all source—the step only warns and continues to upload the empty report, allowing the required matrix check to pass while that crate contributes nothing to the aggregate. This contradicts the six-crate measurement contract and can silently improve or preserve the reported percentage by omitting a crate; treat VALID == 0 as a failed measurement instead.

Document both live data-rule exemptions
Update this maintainer guidance to include the code_coverage exemption: .machine_readable/required-contexts-allow.txt contains two live entries, and the README and developer guide correctly describe both. Calling this a single code_quality entry can lead maintainers to overlook the still-unsatisfied coverage rule when reviewing or removing decision-layer exceptions.

Detect interpolated old-slug-only shell branches
Expand this transfer guard to match shell tests containing ${{ github.repository }}: it currently passes even though governance-reusable.yml still has old-slug-only branches at lines 1771, 1828, and 1892. On metadatastician/standards, those debt-ratchet and JS-runtime bootstrap paths therefore classify the canonical repository as a consumer and use the secondary checkout instead of the PR tree, while this new regression test incorrectly reports that every identity branch was updated.

Keep the live Actions allowlist in scan scope
Remove this blanket exclusion or narrow it to genuinely vendored content: at this commit rhodium-standard-repositories/ contains only the live actions-allowlist interface, including allowed-actions.json, and coordination.k9 explicitly records that both governance workflows consume it. Ignoring the whole directory means Deep Recon never examines changes to this security-policy input even though the accompanying comment incorrectly describes the directory as vendored example code.

What this is
The foundational CI/CD repair, built on top of #1338 (fast-forwarded verbatim — all 31 commits preserved), so this PR supersedes it and #1338 will close automatically when this lands.
State of play (measured, live, 2026-10-25)
inspected=true, 23 producible + 1 app-owned,code_coverageVERIFIED by a successful upload run. One finding remains:rule:code_quality in ruleset 23787415— Code Quality is required by the main gate but not enabled in repository settings, so no result can exist. No file in this tree can fix that; it is declared in the shrink-only allow-list with the owner decision named (enable Code Quality orscripts/remove-unsatisfiable-data-rules.sh --apply --rule code_quality).
