Adds a dedicated Cline OAuth refresh handler.
The generic OAuth fallback submitted form-encoded fields (
grant_type,refresh_token), which Cline rejects. The new handler sends the extension JSON contract withrefreshToken,grantType, andclientType: "extension", then handles the nested response and rotated refresh token.Validation: 3 test files passed; 21 tests passed.
