Copy statement fails silently if append target does not already exist

The behavior I expect is for the table to be created if it does not already exist prior to appending.

One clarification is that it silently failed when no records needed to be inserted. It did of course fail when an insert was attempted and the table did not exist. I still think it should implicitly create the table if it doesn't already exist.
