Some ideas:

Python component as a submodule.
HeuDiCom as well obviously (via Docker)
Orthanc components as distributed, pull it via a script?
Outgoing sending component as a submodule?
Ubuntu vs CentOS? Is the security concern an over kill if everything will be in an hospital IT VM locked down anyway from the outside?
SQLite to store ID substitutions and anonymized confidential information
