It should not have disappeared. That menu entry was added specifically for the “last subtitle line” case, so losing it again after The expected behavior is: select the last subtitle in the grid, right-click that row, and the context menu should still offer “Insert subtitle after current line...”. This is the only way to insert after the final entry from the grid context menu, since the normal “insert between rows” path does not apply at the end. Most likely this got broken by later context-menu cleanup/rebuild work, where the menu items are shown/hidden based on selection state and row position. The command itself should still exist; the issue is probably that the menu item is no longer being added or made visible when the selected row is the last row.
